Application Review: Rolling Review based on applications received 10/1 until full

Notes: Geriatrics specialists can be accepted as a primary care physician. Hospitalists are accepted as primary care physicians but limited to one waiver per site per year.  Facilities must prove 6 months of active recruitment in the 12 months prior to submitting a waiver application. Flex-5 applications are considered for remaining spots beginning April 1.
