Notes: Primary Care and Psychiatrists must fill 22 of the 30 slots. If slots remain available, applications post-marked by March 30, 2011 will be considered. Preference is given to Spanish speakers (or other significantly represented languages). The employer must use a sliding discount-to-fee schedule based on current Federal Poverty Guidelines. One slot is reserved to be used at the discretion of the Arizona Health Administration.
